I am using OOMA to provide phone service to this location, I know it works fine because I tested it at home with a similar setup.  Here is what mono is reporting.

IF is WAN Source is 208.xxx.xxx.xxx port 11xx  Destination(ISP WAN) is 174.xxx.xxx.xxx.xxx port 53xxx UDP.
At home the OOMA box (internal IP) was trying to connect to 208.xxx.xxx.xxx and it appeared to be DNS, which is what I used for the port and it worked; now it’s coming from this IP into my ISP WAN IP?  I tried to configure a NAT rule for this with no luck, any ideas?

How do you know the source port?  It is usually random, and so it is usually "Any" in firewall rules.
